I have SAMSUNG FREEFORM SCH 350. I was in settings on my phone and changed the NAM setting. Phone went off and turned back on and said "intial programming needed" and wanted a 4 diget unlock code. I tryed 9999, 0000, 1234, 5752, last four of my phone number and last four of my MIN number. Nothing worked. Yet none of those worked and after ten trys phone would not let me even try to put any more unlock codes in. Phone told me to wait 60 sec and try again yet after 60 seconds it still would not let me put any more codes in. I called Customer service and was told that you need to go in to corp. METRO PCS store to have METRO PCS unlock your phone. I  took my SAMSUNG FREEFORM SCH 350 and went to METRO PCS CORP STORE today. METRO PCS CORP. STORE  used the code is 2970 and then put 2970 in again and phone was back to normal. . . . I hope this help anyone who was in my situation cause being with out your phone is a pain in the !@$.
