According to the AT&T website, there are a number of ways to find the PUK code (also known as a PIN unlock code) for your phone.
• Access a PUK code online
In order to find your PUK code online, log in to myAT&T. Once you have logged in, there should be a series of tabs at the top of the page. Hover over the tab which says wireless, and click on the link which says PHONE/DEVICE. Once you have clicked on this link, you will be taken to a page provides a number of options. Click on the option which states "unlock SIM card", which can be found under the heading of "my make and model". After you have clicked on that option, you will be presented with your PUK code, and a set of instructions for unlocking your phone.
• Call the customer care helpline
The telephone number for the AT&T Customer Care Helpline is 1800-331-0500. Here, you can talk to an operator, who will be able to help you unlock your phone by providing you with the PUK code for your AT&T cell phone.
• Use the AT&T online chat facility

Please note that if you enter the PUK code incorrectly 10 times in a row, your SIM card will become permanently locked and disabled. This means that you will no longer be able to use your current SIM card. In order to being using your cell phone after locking and disabling the SIM card, you will have to buy a new AT&T SIM card, which can be easily swapped for your old one.
