As far as I know, the puk code is the 2nd level security code of your SIM card, not your phone's. It is invoked if something goes wrong with your SIM code. It should have come in the plastic card, the one you detach the SIM card from. If you haven't kept that card, you should contact your network carrier, to try to get that code back.
