An example of unsuccessful disintermediation done by a company is of Levi Strauss Company. Initially they decided to disintermediate and spent millions of dollars to develop a website on which they could sell directly to the consumers but this effort was unsuccessful and they had to reintermediate by shutting down most of their operations online.
