Most employers have very specific time-off request policies. This is to ensure fairness for all who work there but also that paperwork gets filed appropriately. Typically, then, you have to request time off from work far ahead of time, often two-weeks in advance so that the appropriate changes can be made to your schedule as well to others who may have to take your place. When paperwork is filed properly, the Human Resources department can also try to honor requests from multiple parties that want the same time period off.

If you work in a place like this, with a specific rule set, it can be very difficult to request time off via email. However, you can often get a jump on the date you want off by sending a preemptive email to your manager, supervisor, or Human Resources department head.

Having particular rules and forms that you must follow will actually help you in this regard because you will already know what information they require when you ask for a day off. Generally, though, you will need to give them:

Your name, obviously

Which day or dates you need

The reason you are requesting them off

Your reason can be anything from a personal appointment (like seeing a doctor) or personal time (family in town). Unfortunately, if you work for a small company and someone has more pressing issues that cannot be changed (surgery, for example), you might not be able to get the day off.
