You'll need to carry it in to your Service Provider, because it's not like your phone is asking for a PUK number, The PUK number is actually blocked as well. If your phone requests a PUK 2 code, all you have to do is to call you network provider and request it. But if you phone doesn't ask for a PUK 2 you'll need to carry it in for them to troubleshoot it. But before you take it in try and call them and ask them form another phone if they can send troubleshooting settings to the phone, if they can't they'll ask you to carry it in. Going through all of this is only if you really would like to keep you current phone number, if not, then the easiest thing to do would be to get a new number.
It seems as if some1 was playing with your phone while you weren't around
